AI Health-Forward is intended for personal, non-commercial use by individuals 18 years of age or older. By using the site, you agree not to misuse it, access restricted areas, disrupt its functionality, reverse-engineer our services, or engage in fraudulent activity.
We may suspend or terminate accounts that violate these Terms, and we may refuse service at our discretion where required by law.
Our AI-reviewed Plans organize and summarize your medical records. They are informational tools; they do not replace the judgment of a licensed healthcare provider. Consult a qualified provider before starting any product, especially if you are pregnant, nursing, have a medical condition, or are taking prescription medications.
Checkout requires an AI Health-Forward account. By creating one, you agree to:
Prices are in U.S. dollars and may change without notice; the price shown at checkout is the price you pay. Applicable taxes and shipping are added at checkout.
Basic, Plus, and Premium Plans renew monthly at the rate shown at the time you signed up until cancelled. You may cancel at any time from your account; cancellation takes effect at the end of the current billing period, and you retain access until then.
Plan contents and turnaround times are described at purchase and may be updated from time to time; material changes will be communicated in advance.
All content on this site — including text, images, logos, product designs, and AI-generated summaries — is owned or licensed by AI Health-Forward and protected by intellectual property laws. You may not copy, reproduce, distribute, or create derivative works from our content without prior written permission.
We respect intellectual property and respond to valid takedown notices. Please direct IP concerns to legal@aihealth-forward.com.
When you upload records to use with an AI-Reviewed Plan, you grant us a limited license to process those records solely for the purpose of delivering the plan you purchased. We do not sell your records. Records are handled in our HIPAA-aligned vault and retained only as long as needed for your plan plus any legally required retention period.
You are responsible for ensuring you have the right to share the records you upload.

To the fullest extent permitted by law, AI Health-Forward is not liable for any indirect, incidental, special, or consequential damages arising from your use or misuse of our site, products, or plans. Our total liability for any claim related to a purchase shall not exceed the amount you paid for that purchase in the 12 months preceding the claim.

You agree to indemnify, defend, and hold harmless AI Health-Forward and our affiliates, employees, and suppliers from claims, damages, liabilities, and reasonable legal fees arising from your breach of these Terms or misuse of our products, plans, or site.

These Terms are governed by the laws of the State of Nevada, without regard to its conflict-of-laws principles. Disputes will be resolved in the state or federal courts of Clark County, Nevada, unless applicable law gives you other rights.

If you need to change your shipping address, email us within 1 hour of placing your order. Once an order has entered processing, we may not be able to reroute it.
